A 62 year old male suffered from increasin episodes of chest pain under physical exercise.

The day before and the day after admission to the emergency department he complained about severe chest pain. The ECG showed mild anterior ST-changes and troponin-I test was positive.The coronary angiography revealed a tight stenosis on the left anterior descending artery (figure 1, left upper panel). Primary stenting was performed under proximal protection with Proxis™ (SJM, MN - USA) . Using flow reversal, a large amount of atherosclerotic material was retrieved from the embolic protection system (figure 1, right lower panel). Final angiography showed a TIMI grade III flow, no embolization occurred.
