A 48 48 year old overweight (BMI 31.5 kg/m2) male cigarette smoker with 50 pack years and known hypercholesterolemia experienced a first episode of most painful chest pain. He was brought to the emergency room within 60 minutes after onset of tremendous discomfort. The ECG (figure 1a) revealed an acute anterior wall infarction and the patient was immediately transferred to the catheterization laboratory. Diagnostic coronary angiography showed an acute occlusion of the left anterior descending artery. (figure 2).
